Time to replace this sti cable housing, or is it good for another few thousand miles?

No, it’s not a bulge in the casing; it’s what’s left of the plastic ferrule after the outer part splintered off and the allegedly compressionless housing punched through the ferrule. ‘Tis only a flesh wound :-)

(Replace; that’s a visible bulge in the wrapping. So you’ve got material failure going on.)
